Description:
Pimavanserin is the active pharmaceutical ingredient of Nuplazid®, which was approved by the FDA in 2016 for the treatment of hallucinations and delusions associated with psychosis in patients suffering from Parkinson's disease. Its exact mechanism of action is unclear. The drug exhibits a combination of inverse agonist and antagonist activity at the serotonin 2A receptors (5-HT2A) and, to a lesser extent, at the 5-HT2C receptors in the central nervous system, which is believed to contribute to its antipsychotic activity. Pimavanserin is absorbed in the gastrointestinal tract and is highly bound to plasma protein (approximately 95%). Following the administration of a single 34-mg dose of pimavanserin, the time to maximum plasma concentration is six hours, the half-life is approximately 57 h and the mean (standard deviation, SD) apparent volume of distribution is 2,173 (307) L. Pimavanserin is metabolized in the liver predominantly by cytochrome P450 (CYP3A4 and CYP3A5) and does not cause significant CYP3A4 inhibition or induction. It has a major active N-desmethylated metabolite AC-279, whose half-life is 200 h.
